  • The reference number of the public body's decision (if you have it)
  • The correct fee - view our Fees for review
  • If you believe no fee or a reduced fee applies, supporting information
  • Details of the parts of the public body's decision you are unhappy with and which you want us to review

  • Please download the application form and fill it in.

Please note:

  • If you send your application outside of our office hours, we will treat it as received on the next business day.
  • The date of receipt of your email or letter is not the date of the application. The date of the application is the date when we have received both the application and the correct fee.

What happens next

We will acknowledge your application. We will notify you as soon as we decide whether or not to accept your application.
